REUNIONCare’s monthly Ability Tracker for Memory Skills and Activities of Daily Living (ADL’s) is handled by responding to a series of quick and easy questions using the standard Likert scale.   Our technology teaches family members and caregivers how to become a proactive care team addressing their needs early and personally - and pushes messages directly to the Circle of Care members for a fast and accurate response to the ones you love.

After completing two monthly Ability Trackers and then going forward, REUNIONCare will display an Ability Tracker Comparison in an easy-to-read gauge display for each of the following skills;

  • Orientation Skills

  • Medication use

  • Eating

  • Dressing

  • Brushing

  • Toileting

  • Bathing

  • Getting Around

  • Other Activities and Skills

Additionally, we provide a “What Can We Do To Help?” link for each skill, providing additional resources to help educate first-time caregivers and all Circle of Care members as to what the skills are, why they should care, and what they can do to help their loved one improve their abilities.

Finally, we'll send a friendly reminder each month to complete a new Ability Tracker so you can stay on top of any changes, as soon as possible.
